In order to maintain a Covid-Free environment, reduce face-to-face exposure at our clinics and help keep our patients, staff and community safe and healthy, strict guidelines and protocols recommended by the Philippine Dental Association shall be followed.
As of this time, NO walk-in patients will be accommodated.
Visits are STRICTLY by APPOINTMENT ONLY.
A pre-evaluation/assessment of every patient will be conducted prior to confirmation of appointment to determine if the dentist can proceed with the desired dental procedure.
Kindly fill out the Covid19 Pre-Screening Form below. The same form shall be filled out by the companion, if the patient (who is either a minor, senior citizen or PWD) needs to bring one.
NO ONE shall be allowed in the waiting area other than the patient.
